West Side Collaborative (WSC) Health Service Project Director

The West Side Collaborative (WSC) is a strategic collaboration between nonprofit social service providers. WSC was established to promote the holistic health and well-being of vulnerable populations in Chicago’s at risk West Side communities. The five founding WSC agencies are Deborah’s Place, Facing Forward to End Homelessness, Cathedral Shelter of Chicago, Primo Center for Women and Children and Marillac Social Center.

This position is responsible for the development of the WSC health service project and implementing a medical home model that ensures the coordination and integration of healthcare services to the clients of the WSC health service project. This position is responsible for oversight of health services including: provision of appropriate healthcare referrals, facilitating access to healthcare, advocating on behalf of clients for improved healthcare, facilitating communication among WSC agencies, and the development of on-site direct service and health education programs.

  • Demonstrated experience with systems integration
  • Proficiency with computer spreadsheet programs
  • Proficiency with statistical data collection, analysis and reporting

  • Demonstrated ability to work independently as a self-directed/motivated professional
  • Ability to work flexible hours

Position Description includes:

  • Coordinate work of and provide resources to the WSC health service staff. Develop and implement health education, health care assessments, and health care resources for all WSC agency clients
  • Develop and implement coordinated assessments, health service planning, record-keeping, referral, crisis intervention and advocacy for WSC clients.
  • With WSC leadership, develop and implement programming
  • Responsible for evaluation, data collection, record keeping, reporting and quality assurance for WSC
  • Analyze data trends and statistics and make recommendations for program planning and evaluation
  • Adhere to the highest ethical standards; demonstrate empathic disposition and perseverance; reflects optimistic and positive attitude; conveys commitment to the mission and goals of WSC.

The position is available as of July 15, 2010. The salary is commensurate with skills and experience.

Internships

Deborah’s Place offers unpaid internship / practicum opportunities to bachelor’s- and master’s-level students each academic year and during the summers. Academic year internships usually run from September - May and are 2-3 days/week (depending on the number of hours required by the school). Summer internships usually run from June - August and are 32-40 hours/week. These are overall guidelines; however, Deborah’s Place is flexible and generally conforms with a school’s internship requirements.

Types of Internships and Students:
Students who do their internships here are usually Social Work, Psychology, and Art Therapy, Counseling, Women’s Studies, Adult Education, Leadership and Development, Public Health, or Urban/Community Studies students. Internship positions include: Case Manager, Counselor, Learning Center Intern, Art Therapist, Grants Associate, Human Resources Intern, Volunteer and Goods & Services Associate, Health Services Intern, and Residential Program Intern. Internships may take place at any of our 3 sites, depending on the position. Occasionally, evening and weekend hours are available, again depending on the specific position. Internship Process:
Our process for accepting students for the summer or the academic year is as follows: Deborah’s Place will host several “Intern Information Meetings” in the spring of each year. Prospective interns are strongly encouraged to attend one of these meetings, as this is the beginning of our selection process. At these meetings, students are able to learn about Deborah’s Place, our mission, and our work. They will also hear detailed descriptions of the internship positions available in the summer and fall. At the end of each meeting, students are asked to complete a questionnaire / application that inquires about their interest in an internship at Deborah’s Place, and why they would be a good fit for the internship. After these meetings, DP supervisors will review the questionnaires and then set up one-on-one interviews with students.

Intern Information Meeting Dates for 2010:

Thursday, March 11, from 3:00 PM – 5:00 PM
Tuesday, April 6, 9:30 AM – 11:30 AM
Wednesday, May 26, 5:30 PM – 7:30 PM

Students only need to attend one of these meetings, not all three. Students are encouraged to bring their resumes to the meeting that they attend. All meetings are held at 2822 W. Jackson, Chicago, IL 60612.
